Do external USB TV tuners work in the same way as indoor aerials except that they plug into you pc?

The tuner card does not get a signal. It simply converts your computer into a TV. It still Needs the aerial and as your aerial is rubbish your computer TV will be just as bad. Could you get a signal booster. They are cheap enough and may help.
